also, as part of feedback, gather data from the various teams, and consider aggregating them into trends. the data might be around product (e.g. quality via test trends, customer feedback, metrics on CI and regression tests, etc.) and process (e.g. team processes including how long to stablitize velocity as well as trends on actions teams implement post-iteration retro).

good stuff, would make a great presentation/paper at, oh, say, Agile 20xx! :-)</description>

		<description>What a wonderful piece of work to share. I have to say that I totally agree with Dianna.  We do root-cause meetings around here, but we also balance them with appreciative inquiry.  I believe in working hard on maximizing your strengths, while minding your weaknesses. 

We do our organizational retrospectives prior to our quarterly planning, we use an ORID approach.  We do not do  skip-level meeting, that sounds like a good thing for us to try.
